CHHC is a privately owned and operated Ohio based agency providing home health care to individuals in the comfort of their home. Our purpose is to deliver services that assist individuals in reaching their optimal level of health and independence. Our staff is available 24 hours a day, 7 days a week to coordinate care and instruct families on how to function as caregivers. All personnel are bonded and insured.
CHHC coordinates home health care for physicians, hospitals, managed care organizations and patients of all ages. The company is Medicare Certified and accredited by JCAHO. Services include skilled intermittent home health care on a per hour or per visit basis. Care is provided by Registered Nurses, Licensed Practical Nurses, Occupational, Speech and Physical Therapists, Medical Social Workers and Home Health Aides. Homemaker and companion services are also available on an hourly basis for short or long term intervals.We are now servicing a 28 county area (Ashland, Crawford, Cuyahoga, Erie, Franklin, Fulton, Geauga, Greene, Guersney, Holmes, Lorain, Lucas, Mahoning, Medina, Miami, Montgomery, Morgan, Muskingum, Ottawa, Portage, Richland, Sandusky, Stark, Summit, Trumbull, Tuscarawas and Wayne). Our census includes patients from the Medicare program, private insurance, private pay, state waiver programs and PASSPORT programs, among others.

QUALITY IMPROVEMENT PLAN:Our quality improvement plan consists of an ongoing process of improvement activities and includes periodic samplings of activities not initiated solely in response to an identified problem. Our plan is evaluated annually and revised as necessary. Our client satisfaction survey currently earns a 98.5% satisfaction rating.
