BMT/Malignant Hematology Pharmacist Specialist
provides pharmaceutical care through staffing and clinical rounds. Reviews and monitors physician orders for drug therapy. Serves as a drug information resource to nurses, physicians, and other healthcare workers. Collects statistical data and documents clinical interventions per procedure. Implements identified pharmacy initiatives as applicable to the pharmaceutical care center.
Bachelor of Science Degree in Pharmacy required.
Responds to requests for drug information from health care providers and patients. Evaluates the usefulness of biomedical literature gathered and formulates responses to drug information requests based on analysis of the literature. Provides medication-use education and counsels patients and caregivers.
Builds the information base needed to design a medication therapy regimen. Determines the presence of any of medication problems in patient's current medication therapy.
Designs and provides effective in-service education for physicians, nurses, pharmacy technicians, and other practitioners on medication therapy issues.
Collects statistical data and recommends corrective action based on the results. Observes the MAR for accuracy and proper documentation and reconciles discrepancies. Utilizes and documents existing clinical intervention policies. Participates in Quality Assurance and Performance Improvement activities.
Manages the use of investigational drug products according to established protocols.
Prepares and dispenses medications using appropriate techniques, assuring completeness of the order.
Utilizes pharmacy support personnel effectively and checks the accuracy of the work of pharmacy support personnel. Prioritizes the workload and organizes the work flow, taking into account available resources.
Coordinates, oversees, and participates in direct patient care activities to include clinical services.
Designs and delivers in-service education for physicians, nurses, technicians, and other healthcare professionals regarding drug therapy issues.
Retrieves clinical information for monitoring and review of the medication use process.
Provides concise, applicable, timely literature based responses to requests for drug information from healthcare providers and patients.
Uses effective patient education techniques to provide counseling and education to patients and caregivers.
Assists in the acquisition of non-formulary, compassionate use, and investigational medications.
Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens.
Ensures continuity of care to and from the acute and ambulatory patient care environments.
Precepts students and residents after meeting qualifications.
Complies in all activities with accreditation, legal, regulatory, and safety requirements for a specific practice setting.
Current North Carolina Pharmacy License or eligible for reciprocity required.
