Extremely progressive Level I Trauma Medical Center is seeking a Clinical Pharmacist - Transplant. This is a newly created exciting position with the opportunity for growth within the position and professionally. The scope of care involves all aspects of transplant patients in a unique situation where you will 'follow the patient' / 'beds to meds' program through the entire length of stay. Monitoring, screening, and verification of all drug-related orders
Resolve drug therapy problems
Clinically evaluate and correct problematic drug orders
Educate patients regarding the safe and proper use of their medications
Interact and plan with other health care practitioners best therapy options
Clinically recommend and/or initiate appropriate adjustments to achieve desired medication-related outcomes
Share in the precepting of pharmacy students / residents
Doctor of Pharmacy (Pharm.D.) from an accredited academic program
Minimum of one (1) year of pharmacy practice experience in a similar transplant pharmacist setting or residency training
PGY1, ASHP-accredited residency - highly preferred.
VA Pharmacist licensure or reciprocation eligible
