The hospital provides individualized care directed to the specific needs of each patient. and our center offers 20-bed critical care unit, skilled nursing unit, progressive care unit, medical and surgical units, family birthplace center, and a 24-hour emergency department. Other services include a diagnostic imaging department, company care (a workers compensation program), and an outpatient rehabilitation facility.
We are seeking an experienced Nurse Case Manager. Must have AZ RN Licences, BSN with several years of recent experience in acute care in clinical areas such as Med/Surg, ER, Telemetry or Neuro. Experience in case management as evidenced with CCM designation is a plus. Also required for success is strong knowledge base and experience in Utilization Review, Care Management, Regulatory and Reimbursement Standards. Demonstrated critical thinking skills, problem-solving abilities, effective communication skills and time management skills are required. This is an excellent opportunity to provide Case Management Services in a large hospital setting. Duties will include UR, plans of care, follow up and communication with post hospital facilities, physicians, care providers and insurance companies. The incumbent will provide care coordination for patients based on patient need and payer requirements. Responsible for the quality of clinical services identification and resolution of barriers which hinder effective patient care.
